What does it mean for gear to crash?

I've had gear crash and heard it. But what does it actually mean? What causes it?

It's when it comes out of solution, and it re-crystallizes in the vial. Usually its caused by it getting too cold, or just being too high mg per ml.

Or when there's not enough solvents (BB, EO, oils, etc) in the solution
